CONSENT FOR COLLECTION, USE, AND DISCLOSURE OF PERSONAL INFORMATION
Little Friends Nursery Centre Society (“Little Friends”)
Little Friends is committed to protecting the privacy and personal information of our students and their families in accordance with the Freedom of Information and Protection of Privacy (FOIP) Act.
The personal information we collect is collected under the authority of Section 33(c) of the FOIP Act and is used solely for the purposes of providing early childhood education, ensuring student safety and well-being, and communicating with parents or guardians.
If you have any questions about the collection, use, or disclosure of your personal information, please contact the Board President.
1. PURPOSE OF COLLECTION
I/We understand that Little Friends collects personal information about me/us/my child for the following purposes:
- To provide appropriate care, ensure safety, and meet the educational needs of my/our child
- To comply with regulatory requirements under Alberta’s Early Learning and Child Care Act and Regulation
- To communicate effectively with parents/guardians regarding program activities, emergencies, and child development
- To manage program operations, including billing, attendance records, and administrative functions
- To respond to medical emergencies and maintain health and safety standards
2. INFORMATION COLLECTED
I/We consent to the collection of the following types of personal information:
For Parents/Guardians:
- Contact information (names, addresses, phone numbers, email addresses)
- Emergency contact information
- Payment and financial information
- Family information relevant to the care of the child
For Children:
- Full name, date of birth, and address
- Medical information (allergies, immunization records, health concerns, medications)
- Emergency contact information
- Attendance records
- Photographs and videos
- Special needs or requirements
3. USE AND DISCLOSURE OF INFORMATION
I/We understand that Little Friends may use and disclose the following information:
- To staff members who need the information to provide care and education
- To regulatory authorities as required by law
- To emergency medical personnel in cases of emergency
- To health authorities for public health services and communicable disease control
- To the Government of Alberta pursuant to the Child Care Accountability Program (children are registered in the Child Care Licensing Portal and information is submitted online) and for the purposes of the Child Care Affordability Grant.
- As otherwise required or permitted by law
- Student names used for birthday recognition purposes
- Student names used on artwork, written material or other items to be displayed in the school or at school-sponsored displays in the community.
4. STORAGE, RETENTION, AND SECURITY
I/We understand that:
- Personal information will be stored securely and Little Friends has implemented reasonable safeguards to protect personal information
- Information will be retained for the periods required by law
- Access to information is limited to authorized personnel
5. PHOTOGRAPHS AND VIDEOS
I/We consent to the following use of photographs or videos of my/our child:
- Individual photographs
- Photographs of the classroom and school activities that are used in the school newsletter, year-end scrapbooks, or other purposes within the school community.
I/We understand we can withdraw consent for any specific use of photographs at any time.
Where individual students are identified by name or image and the material intends to be used outside the school community in a public manner (in the media for example), a separate and specific consent is required. You will be contacted prior to this event taking place.
6. PARENT/GUARDIAN ACCESS TO INFORMATION
I/We understand that I/we:
- Have the right to access my/our personal information and that of my/our child
- May request corrections to this information if it is inaccurate
- May withdraw or modify this consent at any time by providing written notice to the Board President
7. CONSENT
**The parties acknowledge and agree that electronic signatures shall be deemed the legal equivalent of handwritten signatures and shall carry the same binding legal effect as a manually signed hard copy of this document
