LITTLE FRIENDS FIELD TRIP PARENTAL CONSENT FORM 2025- 2026

End Time: 11:30 am
Supervising Teachers:
Toddler Room (T/Th, WF) – Kelly Petryk and Wendy Bit
Playroom (T, Th) – Ashleigh Bit and Marie Ulliac
Playschool (MWF) – Ashleigh Bit and Marie Ulliac
Note: If the activity takes place outdoors, the child must have proper footwear (i.e. running shoes or closed toe footwear). It is the parent’s responsibility to ensure their child has sunscreen and a hat.
Inherent Risks:
- All manner of injuries resulting from slip/fall/trip.
- Exposures relating to road/sidewalk conditions, potholes, stairs, parking lots, ramps. Motor vehicle exposure relating to road or parking lot crossings.
- All manner of injuries associated with participation in the planned activity.
- All manner of injuries associated with use of a playground.
- All manner of injuries resulting from use of equipment, materials or facilities.
Parental Consent:
I have reviewed and understand the information provided in the Parental Consent Form. I give my consent for my child to participate in the activity as described in the Parental Consent Form and I agree that the planned activity is acceptable.
I understand and acknowledge that it is my responsibility to ensure that my child has the appropriate footwear and clothing to participate in the activity. If my child is not dressed appropriately he/she may be unable to participate.
I understand and agree that where circumstances arise, such as a change in itinerary or weather conditions, the planned activity may be canceled or rescheduled to another time. I understand that Little Friends and/or my child’s teacher will advise me of any changes and the rescheduled date if circumstances warrant.
If my child requires medical attention and/or in the event of an emergency, I authorize the supervising teachers to seek necessary medical treatment/intervention.
**The parties acknowledge and agree that electronic signatures shall be deemed the legal equivalent of handwritten signatures and shall carry the same binding legal effect as a manually signed hard copy of this document
